Affordable Non-Surgical Alternatives to Rhinoplasty

The inserts sit invisibly in the nose, gently pushing the tip outward to refine the overall appearance of the nose. However, unlike the other options, there are some limitations on using this product. For example, it should not be worn overnight while sleeping. Am I a good candidate for breast lift surgery?

The ideal candidates for a breast lift are emotionally stable, healthy women who have realistic surgical expectations. Other qualifications/characteristics include:

• 18 years of age or older
• Skin that is stretched
• Breast volume that has shrunk over the years
• No intentions of losing a lot weight
• Not currently or have no plans of getting pregnant

A qualified cosmetic surgeon may see best results among women who have small, sagging breasts. If you have heavy breasts, the surgery is still possible; however the results may not last as long due to the stretching of skin and supporting tissue. Some patients think that their breasts can be tightened without scarring, but this is not possible. With advances in technology, the scars will be minimal, but not completely invisible.

What is a concentric breast lift?

The concentric (also known as the “doughnut”) procedure is ideal for women with smaller breasts and minimal sagging. In this procedure, concentric circles (that look like a doughnut) are made around the areola, the doughnut-shaped skin is removed, and your nipple and areola are moved upward. Your outer skin will then be sutured into place around your areola.

With this procedure, sometimes the skin that is sutured to your areola will wrinkle because there now more skin in the area than there was before. Usually, as your skin adapts to the new shape and tissue weight, the wrinkling will go away in a few weeks or months following surgery.

If it seems as if your skin will cause excessive wrinkling, a qualified cosmetic surgeon may opt to make an incision that runs from your areola down to the bottom fold of your breast, where the breast meets the chest. A strip of skin will then be removed along this incision, and both sides will be sutured together. There is still less scarring with this vertical cut than with the common anchor-shaped procedure.

What is the anchor-shaped breast lift procedure?

If you have larger, severely drooping breasts, an anchor-shaped procedure will be more effective. With this procedure, your cosmetic surgeon will draw a key-hole shape above the nipple and areola. He will then draw an anchor shape from the right to the left side of your breast, underneath the key hole. The skin in your upper area of this anchor shape will be removed, in addition to any excess breast tissue. Your nipple and areola will be moved up to the lower portion of the key hole and your skin will be stitched around your areola, vertically down to chest and sideways along the new bottom fold of your breast.

How much pain is associated with the breast lift procedure?

It is normal to experience pain in your breast area for a few days after surgery. The pain will be the strongest during the first 48 hours, but will start to subside each day during your recovery. Your surgeon may prescribe pain medication to help alleviate any discomfort you may feel.

Will I be required to stay in the hospital?

It is not likely. Most breast lift procedures are performed as an outpatient surgery under general anesthesia. In doing this, you will typically be allowed to go home within a few hours after the procedure is completed and comfortably recover in your home. Occasionally, if a significant amount of tissue is removed, you may have to spend the night in the hospital, so the medical staff can monitor your progress.

What are arnica montana and bromelain?

Arnica montana is an herb used to treat swelling, bruising and soreness. There are a variety of types that are taken under the tongue (called sublingual) that are recommended to recover from plastic surgery. Be very careful not to take this herb in its raw form, as this can cause intestinal bleeding and stomach cramping.

Bromelain is derived from the pineapple plant and contains anti-inflammatory properties as well as an enzyme that dissolves proteins. It is effective to control swelling after plastic surgery.

Vitamins A, C, and K are also recommended to aid the recovery process.

Does Lasik Hurt?

LASIK, an acronym for Laser Assisted in situ Keratomileusis, is a surgical vision procedure that corrects the refractive errors of nearsightedness (myopia), farsightedness (hyperopia) and astigmatism. LASIK uses the ultraviolet excimer laser, first approved for use in the United States in 1996, to reshape the cornea without any damage to adjacent tissues.

LASIK and Pain

Inlays & Onlays

If you suspect you need a filling, there are more choices these days than there used to be when your only option was unattractive metal fillings. Today you can choose a white composite fillings or porcelain inlays or onlays. Metal fillings are compressed into the cavity tightly enough to not fall out, but porcelain onlays and inlays are bonded to the tooth, and become part of it thus providing a more stable and durable foundation.

What are inlays and onlays?

Porcelain inlays and onlays are custom-fabricated fillings to help restore teeth that have a cavity. Inlays are smaller, and contained within the cusps, or tips of the tooth. Onlays are made for larger areas of decay and extend over to the outside surface of the tooth.

Advantages of inlays and onlays over metal fillings

• Metal fillings require more of the natural tooth to be drilled out, even if it isn’t decayed so that the tightly packed amalgam will stay put.
• Additionally, with time metal fillings tend to become loose and can even fall out.
• With inlays and onlays, more of the natural tooth can be preserved, since the porcelain will be bonded to the tooth, rather than packed into a space.
• The porcelain of inlays and onlays is matched to your tooth color so more esthetic.

The procedure

Porcelain inlays and onlays typically require two visits to your cosmetic dentist. During the first visit, the damaged or decayed area is very precisely removed and an impression or mold is taken of the tooth. Your dentist will send the mold to a dental lab and they will create the onlay or inlay from dental porcelain.

At the second visit approximately two weeks later, your cosmetic dentist will make sure the inlay or onlay fits properly and then bond it to your damaged tooth.

Advantages of Submuscular Implants

When considering breast augmentation, much attention is paid to the size of the implants and what you’d like your breasts to look like after the procedure is completed. Equally important, though, is where and how the implants are placed, and the long-term effects of this choice.

There are basically two choices when it comes to implant placement: subglandular and submuscular. Subglandular implants are placed beneath the breast tissue, but above the chest muscle. Submuscular implants are placed beneath the chest, or pectoral, muscle. Implants can also be placed partially under the chest muscle.

While subglandular implants are recommended in some circumstances, there are several advantages to submuscular implants. Among these are:

• Lower incidence of capsular contracture
• Less chance of rippling
• Your implant is less visible
• Less interference with future mammograms
• More natural appearance and feel to the breast
• Can provide better results for women with small breasts

Capsular contracture occurs occasionally as a complication of breast implants. When an implant is placed, the body naturally forms a pocket of scar tissue, referred to as a “capsule”, around it. Capsular contracture occurs when this scar tissue tightens abnormally around the implant. It can create a hard, unnatural look to the breast, and can also be quite painful. In order to correct capsular contracture, the breast implant and the capsule must be removed, then the implant is replaced. While capsular contracture can occur with submuscular implants, it’s much less likely than with subglandular placement.

Submuscular placement also allows most, if not all, of the implant to be covered. This reduces the likelihood of a hard-edged, artificial look to your breasts, and also gives it a more natural feel. Rippling is also much less likely with a submuscular implant.

In addition, because the implant is beneath the muscle, it won’t interfere as much with mammography. In a mammogram, pictures are taken of the glandular tissue of the breast. In subglandular placement, the implant is just beneath this tissue, and so can interfere with accurate imagery. With the implant under the muscle, the breast tissue is much more readily visible.

The placement of breast implants is an important decision, one best made between you and your plastic surgeon. If you’re considering breast augmentation, be sure to discuss the options with your doctor to be sure you make the best decision for your body type and the results you wish to achieve from your implants.

Move Over, Botox!

There’s a new drug in town! Reloxin, developed by a French pharmaceutical firm, has emerged as a safe and effective alternative to Botox, and may possibly offer longer lasting results for the reduction of forehead wrinkles.

Botox has long been the main source for reducing forehead wrinkles and frown lines among the patients of those who practice cosmetic medicine. The drug prevents the contraction of facial muscles, which gives the face a more smooth appearance that lasts for several months. Now, the French drug maker Ipsen may have created a possible rival with their anti-wrinkle treatment, Reloxin, according to recent research. The report can been found in the Archives of Facial Plastic Surgery.

Lead researcher Dr. Ronald Moy from the Moy-Fincher Medical Group in Los Angeles said, “Reloxin is as safe and it works as well as Botox,” He went on to say, “I think this will be as popular as Botox, which is the most common cosmetic procedure in the United States.” Moy also added, “Many of our patients are happier and got a more natural look with the use of Reloxin.” Although Reloxin has not yet been approved in the U.S., it has been approved in 23 countries to smooth wrinkles, and has been in used for over 15 years outside the U.S. to treat neurological disorders.

Reloxin differs from Botox in that it contains fewer antigens, allowing the drug to break down more quickly, while it also has less protein in its formula, possibly causing the body to break it down at a slower rate and making it last longer than Botox. In the phase III trials, the effects of Reloxin lasted 13 months.

Another difference between the two anti-wrinkle medications is that Reloxin spreads out more in the tissue than does Botox, which can be more effective in areas such as the forehead. But, according to one expert, the drug’s wider spread into the tissue creates a more tedious task for accuracy in the treatment of areas such as crow’s feet near the eyes. Unless an experienced injector delivers the drug, it may spread to surrounding muscles and cause a droopy eyelid or eyebrow.

During the study, 1,200 patients were treated with Reloxin injections in five different areas of the forehead. Study participants were asked to keep diaries of the effects of their treatments and were given post-treatment assessments after one week to 10 days. Participants reported that Reloxin began to work within one day after treatment. After one week, 95 percent of the patients reported a noticeable response to the treatment. Participants were also assessed after two weeks and one month, with additional assessments following each month after treatment. One month after treatment, about 91 percent of the patients showed a response. Participants in the study received up to five sets of five injections over the 13-month analysis.

A total of 1,052 patients completed the study with 880 of them experiencing a total of 2,838 adverse events. Of these, only about 28 percent were likely related to the treatment and included problems at the injection site, nervous system disorders such as headache, and eye problems including a drooping of the eyelid or brow known as ptosis. The researchers reported that only one patient withdrew from study due to the side effects of the drug.
