LASIK is an ambulatory procedure — you walk into the surgery center, have the procedure and walk out again. In fact, the actual surgery usually takes less than five minutes, and you’re awake the whole time.
Occasionally, the surgeon will give you a mild oral sedative beforehand.
Even though the surgery is relatively quick, LASIK is a very delicate procedure and it’s important to have it performed by a highly skilled surgeon with proper equipment. You also should have someone accompany you to drive you back home afterward.
Your eyes first are anesthetized with special drops, so most people don’t feel pain during LASIK surgery.
The doctor will have you lie down, then make sure your eye is positioned directly under the laser. (One eye is operated on at a time.) A kind of retainer is placed under your eyelids to keep them open — normally, this is not uncomfortable.
The surgeon will use an ink marker to mark the cornea before the flap is created. The flap is then created with either a microkeratome or with a femtosecond laser. Whichever device is used, it is securely attached to your cornea with a suction ring to prevent eye movements or loss of contact that could affect flap quality. During the procedure you won’t actually see the creation of the flap, which is very thin.
An ultra-thin flap is created on the eye’s surface during LASIK corrective eye surgery. After laser energy is applied to reshape the eye, the flap is replaced to serve as a type of natural bandage.
The surgeon uses a computer to adjust the excimer laser for your particular prescription. You will be asked to look at a target light for a short time while he or she watches your eye through a microscope as the laser sends pulses of light to your cornea.
The laser light pulses painlessly reshape the cornea. You’ll hear a steady clicking sound while the laser is operating. You also may smell a mildly acrid odor during the corneal reshaping process.
The higher your prescription, the more time the surgery will take. The surgeon has full control of the laser and can turn it off at any time.
After the procedure is finished, you will rest for a little while. If you’re having both eyes done the same day, the surgeon typically will begin working on your second eye immediately after treatment of the first eye is finished. Some people choose to have their second eye done a week later.
Your surgeon may prescribe medication for any postoperative pain, but many people feel no more than mild discomfort after LASIK. That’s one advantage of LASIK over PRK, which can cause significant eye discomfort for a few days.

LASIK is the most commonly performed refractive surgery procedure. You may hear people calling it “LASIX,” but the correct name is LASIK, which is short for “laser-assisted in situ keratomileusis.”
Why is it so popular? LASIK has advantages over other vision correction procedures, including a relative lack of pain afterward and the fact that good vision usually is achieved by the very next day.
An instrument called a microkeratome is used in LASIK eye surgery to create a thin, circular flap in the cornea. Another, newer way of making the flap is with a laser.
The surgeon folds the hinged flap back out of the way, then removes some corneal tissue underneath using an excimer laser. The excimer laser uses a cool ultraviolet light beam to precisely remove (”ablate”) very tiny bits of tissue from the cornea to reshape it.
When the cornea is reshaped in the right way, it works better to focus light into the eye and onto the retina, providing clearer vision than before. The flap is then laid back in place, covering the area where the corneal tissue was removed.
Both nearsighted and farsighted people can benefit from the LASIK procedure. With nearsighted people, the goal is to flatten the too-steep cornea; with farsighted people, a steeper cornea is desired. Excimer lasers also can correct astigmatism by smoothing an irregular cornea into a more normal shape.

Presbyopia usually occurs beginning at around age 40, when people experience blurred near vision when reading, sewing or working at the computer.
You can’t escape presbyopia, even if you’ve never had a vision problem before. Even people who are nearsighted will notice that their near vision blurs when they wear their usual eyeglasses or contact lenses to correct distance vision.
Presbyopia is widespread in the United States. According to U.S. Census Bureau data, over 135 million Americans were age 40 and older in 2008, and the country is growing older: The median age reached 36.8 in 2008, up 1.5 years since 2000. This growing number of older citizens generates a huge demand for eyewear, contact lenses and surgery that can help presbyopes deal with their failing near vision.
Presbyopia Symptoms and Signs
When people develop presbyopia, they find they need to hold books, magazines, newspapers, menus and other reading materials at arm’s length in order to focus properly. When they perform near work, such as embroidery or handwriting, they may develop headaches, eye strain or feel fatigued.
What Causes Presbyopia?
Presbyopia is caused by an age-related process. This differs from astigmatism, nearsightedness and farsightedness, which are related to the shape of the eyeball and caused by genetic and environmental factors. Presbyopia generally is believed to stem from a gradual thickening and loss of flexibility of the natural lens inside your eye.
These age-related changes occur within the proteins in the lens, making the lens harder and less elastic over time. Age-related changes also take place in the muscle fibers surrounding the lens. With less elasticity, the eye has a harder time focusing up close. Other, less popular theories exist as well.
Presbyopia Treatment: Eyewear
Eyeglasses with bifocal or progressive addition lenses (PALs) are the most common correction for presbyopia. Bifocal means two points of focus: the main part of the spectacle lens contains a prescription for distance vision, while the lower portion of the lens holds the stronger near prescription for close work.
Progressive addition lenses are similar to bifocal lenses, but they offer a more gradual visual transition between the two prescriptions, with no visible lines between them.
Reading glasses are another choice. Unlike bifocals and PALs, which most people wear all day, reading glasses typically are worn just during close work.
If you wear contact lenses, your eye doctor can prescribe reading glasses that you wear while your contacts are in. You may purchase readers over-the-counter at a retail store, or you can get higher-quality versions prescribed by your eye doctor.
Presbyopes also can opt for multifocal contact lenses, available in gas permeable or soft lens materials. Another type of contact lens correction for presbyopia is monovision, in which one eye wears a distance prescription, and the other wears a prescription for near vision. The brain learns to favor one eye or the other for different tasks. But while some people are delighted with this solution, others complain of reduced visual acuity and some loss of depth perception with monovision.
Because the human lens continues to change as you grow older, your presbyopic prescription will need to be increased over time as well. You can expect your eye care practitioner to prescribe a stronger correction for near work as you need it.
Presbyopia Treatment: Surgery
Surgical options to treat presbyopia also are available. One example is Refractec Inc.’s conductive keratoplasty or NearVision CK treatment, which uses radio waves to create more curvature in the cornea for a higher “plus” prescription to improve near vision. The procedure is performed on one eye only for a monovision correction. Studies have shown that NearVision CK effectively treats presbyopia, but the correction is temporary and diminishes over time.
PresbyLASIK is a new presbyopia-correcting surgery now undergoing U.S. clinical trials. This innovative procedure uses an excimer laser to create a multifocal ablation directly on the eye’s clear surface or cornea. This enables vision at multiple distances.
LASIK also can be used to create monovision, in which one eye is corrected for near vision while the other eye is stronger for distance vision.
A highly experimental treatment being studied is the injection of an elastic gel into the capsular bag, the structure in the eye that contains the natural lens. In theory, the gel would replace the natural lens and serve as a new, more elastic lens.
Experiments also have centered on laser treatment of the eye’s hardened lens to increase its flexibility and thereby improve focus.
With the recent introduction of presbyopia-correcting intraocular lenses, some people undergoing cataract surgery may be able to achieve clear vision at all distances.
Also, an elective procedure known as refractive lens exchange may enable you to replace your eye’s clear but inflexible natural lens with an artificial presbyopia-correcting lens for multifocal vision.

In refractive lens exchange (RLE) eye surgery, your eye’s natural lens is replaced with an artificial one to achieve sharper focus.
While RLE technically does not have FDA approval, some surgeons in certain circumstances will use the procedure off label.
This type of use is perfectly legal and sometimes a practical option for elective vision correction surgery, particularly for older people and younger people in some circumstances.
Refractive Lens Exchange for Older People
People who are middle-aged or older may have the beginnings of cataracts that eventually could worsen and cloud the eye’s natural lens. In time, these cataracts could become advanced enough to require cataract surgery and replacement of the eye’s clouded lens with an artificial or intraocular lens.
If you have early cataracts, you could choose to have a refractive lens exchange instead of waiting for the cataracts to advance enough to require cataract surgery. Artificial (intraocular) lenses likely can provide significantly better uncorrected vision at that point, especially if you now require vision correction with glasses or contact lenses.
But keep in mind that you often can receive Medicare or insurance coverage for cataract surgery, which is a necessity, but not for an elective vision correction procedure like RLE. Costs of refractive lens exchange can range from about $2,500 to $4,500 per eye or higher, depending on the type of artificial lens used.
A major appeal of RLE is represented in newer accommodating or multifocal intraocular lenses, traditionally used in cataract surgery, with their ability to restore distance vision as well as improve near vision that enables functions such as computer use and reading for aging eyes. Presbyopia affects all of us beginning at around age 40, when our eye’s natural lens grows more rigid and we lose the ability to focus at all distances (accommodation).
Refractive Lens Exchange for
Younger People
For extreme farsightedness (hyperopia) higher than +6.00 diopters, LASIK is generally not recommended, making refractive lens exchange a possible option.
If a traditional intraocular lens is used, distance vision can be corrected, but reading glasses would be needed. If an accommodating or multifocal intraocular lens is used, reading glasses may be needed infrequently. [See also: Intraocular lenses or IOLs]
While refractive lens exchange is relatively safe, you do need to consider that any surgery has risks that should be discussed in detail with your eye surgeon.
As an example, the FDA notes that the chances of a retinal detachment are slightly higher in people who have undergone refractive lens exchange, compared with the general population. Otherwise, risks are similar to what people undergoing cataract surgery would face. The procedure for refractive lens exchange also is identical to cataract surgery.

“What is a stigmatism?” is a question that eye doctors hear every day, but actually it’s spelled “astigmatism.” Although astigmatism is the most common vision problem, most people don’t know what it is.
Astigmatism may accompany farsightedness or nearsightedness. Usually it is caused by an irregularly shaped cornea (called corneal astigmatism). But sometimes lenticular astigmatism results from an irregularly shaped lens, which is located behind the cornea.
Either kind of astigmatism can usually be corrected with eyeglasses, contact lenses, or refractive surgery.
What Causes Astigmatism?
Astigmatism occurs when the cornea is shaped more like an oblong football than a spherical baseball, which is the normal shape. In most astigmatic eyes, the oblong or oval shape causes light rays to focus on two points in the back of your eye, rather than on just one. This is because, like a football, an astigmatic cornea has a steeper curve and a flatter one.
In regular astigmatism, the meridians in which the two different curves lie are located 90 degrees apart. In irregular astigmatism, the two meridians may be located at something other than 90 degrees apart; or there are more than two meridians.
Regular astigmatism is usually easy to correct (see treatments below), but irregular astigmatism can be complicated and more difficult to correct, depending on the extent of the irregularity and its cause.
Usually astigmatism is hereditary: many people are born with an oblong cornea, and the resulting vision problem may get worse over time.
But astigmatism may also result from an eye injury that has caused scarring on the cornea, from certain types of eye surgery or from keratoconus, a disease that causes a gradual thinning of the cornea.
Astigmatism Treatment
Unless it is extreme, astigmatism can be compensated for satisfactorily with eyeglasses or contact lenses.
If your eyeglass or contact lens prescription contains three parts rather than one, your eyecare practitioner has found some astigmatism in one or both of your eyes. A prescription with three parts looks like this: -2.75 -1.25 x 90.
- The first part indicates your main spherical correction, meaning the amount of power (in diopters) required in a lens to sharpen your visual acuity to an acceptable level, usually 20/20. In this example, the person has myopia and requires a negative (concave) lens to correct it.
- Part two shows the extent of the astigmatism in diopters. Again, the minus sign means a concave lens is needed.
- Part three is the axis (in degrees) of the cylinder required to bend certain light rays to compensate for the cornea’s oval shape.
Many people with astigmatism believe that they can’t wear contact lenses or that only rigid contact lenses (RGPs, also called GP lenses) can correct astigmatism.
This was true many years ago, but now soft toric contact lenses can correct astigmatism. Toric lenses have a special correction built into them and may also contain a prescription for nearsightedness or farsightedness if you need it.
While soft torics work well for many people, if you have severe astigmatism, you’ll likely do better with RGP contact lenses or eyeglasses. Your eyecare practitioner will advise you.

Happy with your health insurance? Its probably because you’re healthy and don’t really need it. But when you do get sick, you may discover your insurance doesn’t cover as much as what you thought.
Just when you have the least physical and emotional ability to cope, you find yourself dealing with a financial and bureaucratic nightmare of claims that are refused, and procedures and medications that are denied by an anonymous insurance administrator telling their doctor what they can and cannot prescribe.
Saying the U.S. has the best health care in the world doesn’t make it so.
The World Health Organization ranks the U.S. health-care system 37th, between Costa Rica and Slovenia, behind three South American countries and well behind the much-maligned UK and Canadian systems.
While it’s true we have some of the best technology, research and training for doctors and nurses, if Americans can’t access them, it doesn’t make much difference how great it is. French cuisine is arguably the best in the world, but try telling that to the starving 18th century French peasants who because of their hunger and other sufferings, started the French revolution.
While false fears run rampant about “pulling the plug on Grandma” and government funds for abortion, there seems to be little regard for people between the age of 0 and 65 who are dying, suffering or going bankrupt because they just can’t afford the health care they need.
In this, one of the wealthiest countries in the world, every child, every person, should have access to health care. No parent should have to wait until their child gets so sick that the emergency room is the only option. No one should have to divorce an ailing spouse in order to avoid bankruptcy or poverty. No one should die because they can’t afford to be treated. Access to affordable health care, like freedom and democracy, is a moral issue.

Many Costa Rican hospitals and clinics have received certification that puts them on par with U.S. medical centers, a move that national competitiveness authorities know will boost this country’s profile as a prime destination for medical tourism.
Following a six-month review process, the American Association for Accreditation of Ambulatory Facilities (AAAASF) has certified several private surgical and ambulatory centers, such as UNIBE hospital, And top JCI accredited hospitals, or soon to be accredited, include CIMA Hospital, Hospital Clinica Biblica, and La Catolica Hospital.
“The international accreditation of health services allows us to position ourselves among countries such as the United States, where patients are looking for certified qualified,” said Dr. Jorge Cortés, president of the Council for the International Promotion of Costa Rican Medicine (PROMED).
The vast majority of Costa Rica’s medical tourists come from the United States, whose population in many cases lacks affordable health care. Many procedures here are as little as one-third the cost, or less.
Nearly 13 percent of the medical tourists who visit Costa Rica seek cosmetic surgery procedures, while another 35 percent come for dental work, according to a study by the Competitiveness Ministry.
