Happy with your health insurance? Its probably because you’re healthy and don’t really need it. But when you do get sick, you may discover your insurance doesn’t cover as much as what you thought.
Just when you have the least physical and emotional ability to cope, you find yourself dealing with a financial and bureaucratic nightmare of claims that are refused, and procedures and medications that are denied by an anonymous insurance administrator telling their doctor what they can and cannot prescribe.
Saying the U.S. has the best health care in the world doesn’t make it so.
The World Health Organization ranks the U.S. health-care system 37th, between Costa Rica and Slovenia, behind three South American countries and well behind the much-maligned UK and Canadian systems.
While it’s true we have some of the best technology, research and training for doctors and nurses, if Americans can’t access them, it doesn’t make much difference how great it is. French cuisine is arguably the best in the world, but try telling that to the starving 18th century French peasants who because of their hunger and other sufferings, started the French revolution.
While false fears run rampant about “pulling the plug on Grandma” and government funds for abortion, there seems to be little regard for people between the age of 0 and 65 who are dying, suffering or going bankrupt because they just can’t afford the health care they need.
In this, one of the wealthiest countries in the world, every child, every person, should have access to health care. No parent should have to wait until their child gets so sick that the emergency room is the only option. No one should have to divorce an ailing spouse in order to avoid bankruptcy or poverty. No one should die because they can’t afford to be treated. Access to affordable health care, like freedom and democracy, is a moral issue.

Many Costa Rican hospitals and clinics have received certification that puts them on par with U.S. medical centers, a move that national competitiveness authorities know will boost this country’s profile as a prime destination for medical tourism.
Following a six-month review process, the American Association for Accreditation of Ambulatory Facilities (AAAASF) has certified several private surgical and ambulatory centers, such as UNIBE hospital, And top JCI accredited hospitals, or soon to be accredited, include CIMA Hospital, Hospital Clinica Biblica, and La Catolica Hospital.
“The international accreditation of health services allows us to position ourselves among countries such as the United States, where patients are looking for certified qualified,” said Dr. Jorge Cortés, president of the Council for the International Promotion of Costa Rican Medicine (PROMED).
The vast majority of Costa Rica’s medical tourists come from the United States, whose population in many cases lacks affordable health care. Many procedures here are as little as one-third the cost, or less.
Nearly 13 percent of the medical tourists who visit Costa Rica seek cosmetic surgery procedures, while another 35 percent come for dental work, according to a study by the Competitiveness Ministry. Because of lower costs of medical supplies in Costa Rica, as well as a lower cost of living, prices are driven down compared to American providers.
When it comes to dental, high costs are very common. Dental coverage involves out-of-pocket expense, and the low prices and state-of-the-art dental offices in Costa Rica has proven very popular for patients and insurance companies alike.
Compared to the U.S., Margie Cardot, 49, learned of the huge savings in Costa Rica from her Blue Cross Blue Shield provider. With their encouragement, she wasted little time in getting on a plane for a short ride to San Jose for some of the best dental work of her life. The resulting dental work and cost savings was phenomenal and well worth the trip.
She had 6 crowns and a bridge replaced, along with a root canal and fillings. The dental work would have easily cost her $8,000 out of pocket back home, but she paid just $2,250 after insurance.
In another case, Morris Stewart of Shenendoah, Virginia. would have spent his entire $10,000 deductible allowed through his insurance policy if he had his leg surgery performed earlier this year near his home. For this reason alone, Stewart, 61, had planned to wait until age 65 and became qualified under Medicare before undertaking the surgery.
But, upon reading about medical tourism in the insurance company’s annual report, the retired businessman flew to Costa Rica and paid about $3,700, including all travel expenses. He even flew for free, using frequent flier miles.

Because of Costa Rica’s prominence in medical travel, it has become a major destination for medical and dental procedures of all kinds.

Common cosmetic procedures
- Laser resurfacing uses a laser light to heat, damage, or destroy the upper layers of the skin. This can tighten the skin or cause new skin to grow. It is typically used to remove or improve the appearance of wrinkles, brown spots, shallow scars (from acne, surgery, or trauma), and sometimes, the appearance of small veins. Lasers also can be used to remove unwanted hair. Tattoos are removed with surgery or a laser that breaks down the color into fragments. These fragments are removed by your immune system.
- Chemical peel uses a chemical solution to remove the top layers of skin, allowing new skin to grow. It is most often used to remove wrinkles, superficial skin growths, shallow scars, pigment changes in the skin, and other skin problems.
- Dermabrasion uses a fine wire brush or a diamond wheel with rough edges to remove the upper layers of the skin, allowing new skin to grow. It may be used to treat acne scars and wrinkles around the mouth or to treat an enlarged nose (rhinophyma) caused by rosacea (facial inflammation and redness).
- Collagen and other filler injections are used to smooth wrinkles or pitted scars in the skin. They are also used to make the lips fuller. When injected under the skin, a filler raises or puffs up that area. This goes away with time.
- Botox injections use a protein that weakens the muscles of the face to reduce wrinkles. The effect is temporary, lasting 3 to 4 months on average.
- Sclerotherapy reduces or removes the appearance of small varicose veins and spider veins. In sclerotherapy, a chemical is injected into a vein to damage and scar the inside lining of the vein, which causes the vein to close.
- Electrolysis permanently removes unwanted hair such as facial hair. In electrolysis, an electric current is applied to the hair root, which heats and destroys the hair follicle. Hair is not able to grow back in this area.
- Medical tattooing uses tattoo ink to change the look of a medical problem. For example, it can be used after breast surgery to create the look of a darker nipple and areola. Medical tattooing can also be used to darken white skin patches (vitiligo) and scarred skin.
Common cosmetic surgeries
- Face-lift (rhytidectomy) removes excess skin and fat and tightens the muscles to smooth the face. Improvement is especially seen along the jawline and in the neck.
- Eyelid surgery (blepharoplasty) removes excess skin, fat, and muscles from the eyelids to correct drooping eyelids and remove bags and puffiness. Surgery may be done on the upper and lower lids.
- “Nose job” (rhinoplasty) reshapes the nose by removing or rearranging its cartilage and bones. Rhinoplasty is done to change the appearance of your nose, improve breathing and nasal function, or achieve a combination of both.
- Breast enlargement (augmentation) places an implant under the breast tissue or the chest muscle to make the breast larger.
- Breast reduction removes excess breast tissue and skin to reshape and lift the breast. Women may seek breast reduction to change their appearance, reduce back pain, and reduce limitations of activities caused by large breasts.
- Liposuction uses suction to remove stubborn fat that has not gone away with a healthy weight loss diet and exercise. For women, this can be on the outer or inner thighs, neck, upper arms, belly, knees, and hips. For men, this can be on the waist, belly, neck, breasts, or back. The purpose of liposuction is to reshape the area rather than to reduce body weight. If you are trying to lose weight, it is not a substitute for exercise and a balanced diet. If you regain weight after having liposuction, the fatty bulges that were removed may return.
- “Tummy tuck” (abdominoplasty) removes excess skin and fat from the abdominal area and tightens abdominal muscles. When a large weight loss or several pregnancies leave behind extra folds of skin, some people use this surgery for a flatter, smoother stomach. As with liposuction, the purpose of a tummy tuck is to reshape the area rather than to reduce body weight.
- Varicose vein surgery removes large varicose veins. During varicose vein surgery, an incision is made over the varicose vein, and the vein is tied off and either left in place or removed.
- Hair transplantation surgery reduces the appearance of bald spots or hair loss by moving hair from one part of the head to another.
Although death is very rare after liposuction, a tummy tuck, or any cosmetic surgery, it can happen. If you are having a large amount of fat removed, are obese, have more than one procedure done at the same time, or have health problems, your risks go up. You may be watched closely for several hours or overnight after liposuction or a tummy tuck. Staying in the hospital is sometimes best.1

Botox is the brand name of a toxin produced by the bacterium Clostridium botulinum. In large amounts, this toxin can cause botulism, which you probably associate with food poisoning. Despite the fact that one of the most serious complications of botulism is paralysis, scientists have discovered a way to use it to human advantage. Small, diluted amounts can be directly injected into specific muscles causing controlled weakening of the muscles.
The FDA approved such usage in the late 1980s upon the discovery that Botox could stop ailments like blepharospasm (uncontrolled blinking) and strabismus (lazy eye). Cosmetic physicians have been using Botox for years to successfully treat wrinkles and facial creases. In April 2002, Botox gained FDA approval for treatment of moderate-to-severe frown lines between the eyebrows – called glabellar lines. However, Botox is often used for other areas of the face as well.
How Does Botox Work?
Botox blocks signals from the nerves to the muscles. The injected muscle can no longer contract, which causes the wrinkles to relax and soften.
It is most often used on forehead lines, crow’s feet (lines around the eye) and frown lines. Wrinkles caused by sun damage and gravity will not respond to Botox.
How Is the Procedure Performed?
The procedure takes only a few minutes and no anesthesia is required. Botox is injected with a fine needle into specific muscles with only minor discomfort. It generally takes three to seven days to take full effect and it is best to avoid alcohol at least one week prior to treatment. Aspirin and anti-inflammatory medications should be stopped two weeks before treatment as well in order to reduce bruising.
How Long Does a Botox Injection Last?
The effects from Botox will last four to six months. As muscle action gradually returns, the lines and wrinkles begin to re-appear and wrinkles need to be re-treated. The lines and wrinkles often appear less severe with time because the muscles are being trained to relax.
What Are the Side Effects of Botox?
Temporary bruising is the most common side effect. Headaches, which resolve in 24-48 hours, can occur, but this is rare. A small percentage of patients may develop eyelid drooping. This usually resolves in three weeks. This development is usually caused by migration of the Botox and for this reason, you shouldn’t rub the treated area for 12 hours after injection or lay down for three to four hours. There have been no allergies associated with Botox to date.

The FDA issued a requirement for black box labeling on Allergan, Inc. Botox® and Botox Cosmetic®, Solstice Neruoscience’s Myobloc®, and Ipsen’s Dysport®, the newest formulation of botulinum toxin. Hospitalizations and deaths due to botulism poisoning have been reported, although a WebMD article of April 30, 2009 reports that the “Deaths among adults are suspected but could not be confirmed.” This report also states that no serious side effects have been confirmed due to cosmetic uses of Botox® and Myobloc®.
The key word in the above statements is “confirmed” as there have been a large number of reports filed. The decision to require the black box warning resulted from hospitalizations, including deaths of children with cerebral palsy who received botulinum toxin to treat muscle spasms. While reports indicate that the danger is associated with the higher doses used in therapeutic treatments, other reports suggest that cosmetic uses, especially the off-label uses have resulted in serious adverse reactions.
How bad can it get?
The FDA’s Adverse Event Database (AERS) reports 180 adverse events, of which there were 16 deaths. Many more are posted online from the European Union (EU). As of November 2005 693 cases were reported throughout its 27 member nations of which there were 17 deaths due to aspiration pneumonia.
The most common adverse events result from the toxin spreading from the injection site to distant sites where it has caused incontinence, drooping eyelids, double or blurred vision, difficulty talking, hoarseness, dry mouth, muscle weakness, breathing difficulties, and dysphagia (difficulty swallowing,) which leads to aspiration pneumonia due to weakness of the epiglottis and esophageal muscles. Weakened esophageal muscles can’t prevent reflux, and a weakened epiglottis doesn’t close off the airway sufficiently and allows stomach contents, or food or drink to enter the upper airways and lungs, a potentially fatal condition known as aspiration pneumonia.
Approved uses of botulinum toxin
There are two approved uses of botulinum toxin drugs. The first is therapeutic, primarily to treat cervical dystonia (uncontrolled muscle spasms in the neck and shoulder.) The second is cosmetic, to reduce frown lines between the eyebrows, or to treat excessive underarm sweating. Most cosmetic use of Botox® is not FDA approved and is considered off-label. Off-label uses of Botox® include injections in unapproved sites as well as in multiple sites.
According to the FDA, if the drug is used as descried on the label the likelihood of adverse reactions is near zero. Myobloc® is approved only for cervical dystonia. Because of dosage differences and type differences (Botox® is a type A and Myobloc® is a type B toxin) switching from one manufacturer’s formulation to another is a potential risk that can result in an overdose with the toxin spreading to unintended muscles.
The black box warning is accompanied by requirements to inform users in writing that botulinum toxin products have the potential for distant spread at the time of their injection. Manufacturers are to warn doctors and patients about the dangers of overdosing by substituting different botulinum products. Manufacturers are also to follow the outcomes of children and adults who receive their botulinum toxin drugs for involuntary muscle movement, and to submit safety data to the FDA.

BOTOX® injections have been the number one non-invasive procedure for several years now – for both men and women, and in many age groups. That is not surprising since the BOTOX® results are visible quickly and the procedure is short and simple.
What is BOTOX®?
BOTOX® is the trade name for a substance made from the botulinum toxin. That toxin is produced by the bacterium Clostridium botulinum and when ingested in its natural form it is highly toxic. But in its purified form as BOTOX® it is not toxic at all.
BOTOX® has been used for years to treat conditions involving muscle dysfunction, such as Blepharospasm (twitching eyelids) and multiple sclerosis (MS). When a very tiny dose of the purified substance is injected into a specific muscle, it temporarily paralyzes that muscle. It prevents that muscle from contracting by blocking messages sent by the brain telling it to contract. This helps an MS patient, for example, by cutting down on involuntary movements and enabling more coordinated movements.
BOTOX® began getting FDA approvals for medical uses in 1989 and in 2002 was approved for cosmetic use.
BOTOX® For Cosmetic Use
If you make repeated facial expressions, those muscles work a lot. They contract a lot and squeeze the skin above them together, forming lines or wrinkles. Over the years, those lines and wrinkles become permanent and are reinforced daily by the repeated muscle use.
BOTOX® is most often injected into the frown muscle (the corrugator supercillii). When this small muscle is unable to contract, you do not frown. The vertical frown lines become smoother and the face takes on a more relaxed look.
BOTOX® is also used to smooth out horizontal forehead lines, crow’s feet and neck bands. Some wrinkles are caused by sun damage and some by the daily pull of gravity, and these types of wrinkle do not respond to BOTOX® treatments.
The BOTOX® Procedure
It takes just a few minutes to perform. You may be asked to avoid alcohol for a week or so before your treatment. To prevent bruising it is best to take no aspirin or anti-inflammatory medications for about two weeks before treatment.
No anesthetic is required, as the injection needle is so very fine. There may be some minor short-lived discomfort.
You will see results after a couple of days and they will last for up to four months. Gradually the effect wears off and a repeat treatment must be done.
