What is the Difference Between Traditional LASIK and Custom LASIK?

An excellent question that probably isn’t asked often enough. Folks see the word ‘LASIK’ and generally assume all procedures are the same. However, they are not. If you really want to do your homework and be knowledgeable about a surgery that affects your eyesight, learning the difference is important.

First of all, LASIK is an acronym that stands for Laser-ASsisted In situ Keratomileusis. The basic procedure behind LASIK is simply that a laser is used to create a flap in the cornea. The cornea is then reshaped using one of different methods to return your eyesight to as close to 20/20 as possible.

An aberration of a lower order would be near-sightedness, farsightedness or astigmatism. An aberration of a higher order would be night blindness and other night vision problems such as seeing halos around lights. (Aberration is just a fancy doctor word for vision problem.) Most ophthalmologists agree that the traditional form of LASIK surgery is better suited to resolving the lower order vision problems, while custom LASIK can treat lower order and higher order problems equally well.

Now, to the differences. First, let’s look at traditional LASIK. Your existing vision prescription is the basis for the laser settings used to reshape the cornea, which is what defracts light so you see clearly (or not so much). Unfortunately, this method is not pinpoint accurate, so the results may be less than 20/20.

Custom LASIK, on the other hand, uses the relatively new wavefront technology. Wavefront technology is used much like a roadmap. The technology maps out an exact route for the laser to take so the results are predictable and much more accurate.

Both traditional and custom LASIK procedures are fairly quick, with recovery procedures and timelines being somewhat similar. Most patients are back at work within a day.

Keep in mind that this is a very basic description of the two types of procedures. You are strongly encouraged to discuss all of your laser vision correction options in detail with the LASIK surgeon you choose. If you are completely satisfied with the answers you get to all your questions, consider seeking a second opinion.

As with any field, not all laser vision correction surgeons are created equal. Some use old, out-of-date equipment that may mean a higher risk factor for you. Others use modern technology with state-of-the-art equipment and have a staff that is fully trained to provide you the best possible outcome and care possible.

This is your eyesight we’re talking about. Having what is considered for many to be an elective surgery is not something to be taken lightly. Go in knowing as much as you can, ask all your questions and be sure you feel completely comfortable with the surgeon you select.

No More Tears: LASIK Surgery and Dry Eyes

Many people live with a condition called “dry eye” on a daily basis. Dry eye occurs when a person’s eyes do not generate the proper amount of tears to keep the eyes adequately lubricated. People with dry eye may have symptoms such as:


• A feeling of scratchiness in the eyes
• A burning sensation
• Large amounts of mucous
• Sensitivity to wind and smoke
• Increased light sensitivity

Oddly enough, people with dry eye may also experience excessive tears as the lacrimal glands, which produce tears, try to compensate for the lack of lubrication.

Laser-Assisted in Situ Keratomileusis, also known as LASIK eye surgery, is a procedure to correct vision in patients who live with:


• Myopia or nearsightedness
• Hyperopia or farsightedness
• Astigmatism

While most people who undergo this surgery by qualified, experienced surgeons end up with 20/20 vision or better without complications, LASIK can leave some patients with the dry eye condition. This is especially true if the patient had dry eye prior to the surgery. In those cases, candidate screening was inadequate and the surgery should not have been done.
How Does LASIK Cause Dry Eyes?
During LASIK eye surgery, the surgeon cuts a thin corneal flap so the cornea can be reshaped underneath the flap using a laser. Creation of the flap inevitably cuts some of the corneal nerve fibers related to tear production.

The lacrimal glands are beneath the upper eyelids and just below the eyebrows. They produce tears which flow through a drainage system and flush debris from the eyes. When these nerves are disrupted by flap creation, tear production is reduced and this happens in all LASIK procedures. In most cases the nerves regenerate within a few months and tear production becomes normal. In the meantime, moisturizing eyedrops are prescribed.

Special lubricating drops may also be prescribed for people with mild dryness prior to LASIK surgery to minimize potential complications. Usually dry eyes are a disqualifying condition for LASIK. But an eye surgeon will assess each individual and make a judgment call.
Dry Eye Syndrome
In a few cases, the nerves do not regenerate well and that person suffers from Dry Eye Syndrome. This is a chronic condition rather than a temporary side effect of LASIK. It can also exist quite unrelated to LASIK.

It can be treated or managed. Prescription eyedrops can help. Your eye surgeon may insert plugs into the tear duct openings (the puncta; singular is punctum). These openings are in the inner corners of the eyes and allow tears to drain from the eyes into the nasal passages. Punctal plugs reduce tear drainage to keep the eyes better lubricated.

If you are considering LASIK eye surgery, be sure to inform your LASIK surgeon of your medical history, including any previous diagnosis of dry eye, as well as medications you are currently taking. This will help the surgeon provide you with the most effective, comfortable post-op experience possible.

A Glimpse at Laser Eye Surgery

If, like many people who suffer from imperfect vision, you have had enough of breaking your glasses or losing your contact lenses, then you may have considered undergoing laser eye treatment. Offering you the chance to improve your quality of life, as well as giving you more freedom, it is becoming an increasingly common procedure to have.

A large proportion of people worldwide have already opted for laser vision correction. According to the testimonies, it has not only served to enhance the patients’ vision, but has also provided them with more independence and confidence in life. But, how does it work and what are the different treatments available?

Essentially, the procedure is open to those people who suffer from acute short or long-sightedness, or astigmatism – this where your cornea is an oval rather than spherical shape, meaning images are blurred at most distances.

Your optician will be able to determine whether you should undergo laser eye treatment. If they believe that this is the case, then most procedures can be carried out very quickly and efficiently, and are performed using a local anaesthetic.

Generally speaking, there are two types of laser surgery available; LASEK (Laser-Assisted-Epithelial Keratomileusis) and LASIK (Laser-Assisted-In-Situ Keratomileusis). Each procedure is carried out whilst the patient is awake; eye drops are applied to numb the eye.

With LASEK laser eye treatment the top layer of the cornea, the epithelium, is removed before an excimer laser is used to re-sculpt the cornea. Once this is completed, the surgeon then replaces the epithelium on the eye’s surface. To keep it in place a soft contact lens is inserted, which must be kept in for a few days after the treatment.

LASIK eye treatment, on the other hand, is the most commonly executed procedure globally and is suitable for a wide number of people. Time wise, it is also extremely efficient since it is possible for both eyes to be treated at the same time, whilst it only takes around 10 to 15 minutes to complete. Patients have additionally reported less discomfort following the treatment compared to other types of laser surgery.

The difference between LASIK eye surgery and LASEK is that the former does not remove the epithelium, but instead creates a flap using specific cutting techniques. And, once the laser treatment has been completed, the flat is simply replaced, which helps it to heal more naturally.
